If you are receiving services through waivers, such as Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC), or Developmental Disabilities (DD), 18 years old and above, and living in your home, you can receive individualized home supports. This program provides you with support and/or training to live independently in your home or community.
Our caregivers can provide direct supervision, cueing, guidance, instruction, maintenance, incidental assistance with ADLs, and assistance with coordinating community living activities. At the same time, we have professionals who can provide instructional services, training you in developing community living skills identified in your assessment. Training includes skill-building to acquire, retain, and improve your experience in the community.
